The Family Health Team is unable to keep a list of patients needing a Family Physician or Registered Nurse Practitioner (NP); however, you are encouraged to register with Health Care Connect by calling 1-800-445-1822 for the Health Care Connect patient database Health Care Connect - Requirements for Registering - MOH
In the meantime, the Family Health Team has a number of healthcare professionals who can help you manage your health. Our team provides individual appointments and group programming in targeted acute care, chronic disease management, health promotion and disease prevention for everyone. Some of the healthcare professionals that you may interact with include Registered Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitian, Mental Health Navigator, Registered Social Worker, Rapid Access Addiction Medicine, Medical Receptionists, and more.
If you are already a registered patient at the Espanola & Area FHT, you may be able to book an appointment online with one of our healthcare professionals: Check In | Online Booking
Many programs/services are available to you even if you do not have a Physician/NP or have a Physician/NP outside of our area.
