Family Health Teams

Programs and Services

 

The Family Team has a number of health care professionals who can help you manage your health. They include Registered Nurses, Registered Practical Nurse, Registered Dietitian, and Registered Social Worker.


Many programs/services are available to you even if you do not have a family physician or have a family physician outside of our area.
Members of our team provide individual appointments and group programming in targeted acute care, chronic disease management, health promotion and disease prevention for everyone.
